New Patient Registration

If you live in our Ball Tree Practice Area and would like to register with us, please book a registration/health check appointment for all adults (age 16 plus).  Please only use the Ball Tree registration forms downloadable below.

To register you will need to provide formal documents to use as verification of both your identity and residency - full details are on our Registration Form.

Examples include: driving licence, utility bill, local authority rent card, recent bank, building society or credit card statement. These must be in your name showing your registered address.  We are not allowed to fully register anyone until we have proof of ID and residency. [PDF]

Please let us know via our registration form if you have any particular needs regarding communications or information. We will our best to support you.   

You will be registering with the practice rather than an individual GP.  However, every patients at has a 'Named GP' responsible for their overall care at the surgery.  Ask us about this when you register.  You can book your appointments with a GP of your choice but we recommend that wherever possible you try to see the same GP for ongoing problems.  If you prefer a particular type of doctor e.g. female for a female patient we will note this and do our best to respect your choice.

Patients applying to join the list are not discriminated against on grounds of race, gender, social class, age, religion, sexual orientation, disability or medical condition.  We expect our teams to be respected by our patients in the same fashion.  Please read through our Practice Leaflet for more information.
